When it comes to Chronic Disease, Cardiovascular illness is the leading cause of death around the world. Itheum was approached by a leading outcomes based research institute to co-design a proactive monitoring program that looks at how various metrics like like blood pressure, diet, activity and stress relate to poor Cardiovascular health.

This program had to include only evidence based data (see 'Smart Data types used section') and thanks to the flexibility of Itheum's data collection toolkit it was designed, built and released within 30 days of the initial idea.

Users were on-boarded and coordinated via the secure Telegram Chat app, custom made education modules were presented to the user to set expectations and data was collected and with consent shared back to the research institute who commissioned the program.

The Red Heart Challenge is now undergoing an upgrade based on outcomes after the initial study was concluded.

After having a baby one in seven to ten mothers develop postnatal depression (PND) and comes on within 12 months of having a baby, usually during the first few weeks or months. It can start slowly or suddenly, and can range from very mild and transient, to severe and lingering. For most women, it passes quickly, but others will need professional help.

The Post Natal Depression Tracker was built on Itheum as a means to help new mothers keep a lookout for early signs of PND as they are discharged from hospital.

The program uses evidence based questionnaires to monitor PND levels, this program also utilizes the Personal Data Vault to secure highly sensitive responses from users relating to mental health and self harm.

The program also utilizes the Support Team feature, which consists of multiple, nominated support contacts who get alerted when PND levels are of concern.

This program collects insights about users' exposure to the global blockchain DeFI ecosystem. People globally and from all ages, skills levels and experience use DeFI tools today and we need to understand current usage trends to design better for mass adoption.

Data is collected via a structured data collection program that runs over Telegram and aims to collect data such as DeFI apps exposure, DeFI pain points, investment preferences, barriers to entry and DeFI DApp sentiment.

Data produced from this program can be used to better design DeFI user experiences and DApps and tooling. The target buyers of this dataset will be blockchain research institutes, DApp developers, blockchain protocol developers and CeFI/DeFI organizations.

Normal blood pressure is around 120/80. If your blood pressure is regularly over 140/90, you have high blood pressure. This condition, also known as hypertension, is the most important known risk factor for stroke. It can quietly damage your body for years before symptoms develop and therefore proactive methods of identifying and controlling is required before it develops into a serious, lifelong chronic disease.

Itheum was commissioned by a leading Cardiovascular specialist to build a program that allows for the:

1. Proactive identification of new patients with hypertension
2. Finding the correct medication + dosage for patients recently diagnosed with hypertension

The program has already produced some key success stories:

• One user who was misdiagnosed with a nerve condition, being correctly re-diagnosed and treated for hypertension
• One user having an ineffective medication plan being detected and corrected early
• Multiple users with hypertension taking a more active role in managing their condition

Epilepsy is a chronic condition that causes repeated seizures and is the most common chronic (long term) neurological disorder in pets. In most cases epilepsy is a lifelong disease and needs to be 'controlled' with medication in order to prevent serious damage to your pets.

As with Epilepsy treatment in humans, once your pet is diagnosed, VETs need to try various medications and dosages to bring your pet's condition under control. This process can take many months and involve costly blood tests and vet visits. The primary reason why it takes this long to find a treatment plan is basically a lack of actionable data about the frequency of seizure episodes

The Pet Seizure Monitor program was built on the Itheum platform to give pet owners the ability to monitor their pet's seizure activity and share the data with their VETs. The actionable data is then used to adjust the medication plan and this results in far less visits to the VET and better treatment for your pets.

Proactive monitoring of wounds is required to identify complications and issues early. Typically a patient is required to visit a clinic multiple times for this monitoring to happen.

This program was built on the Itheum platform to allow patients to remotely monitor their wound healing progress and report back to their doctors with more actionable data.

The program allows for:

• Photos of the wound to be taken and uploaded
• Evidence-based questionnaire to log healing progress (e.g. pain levels)
• User to take a more active role in their recovery
